Services Provided

MYCHART PATIENT PORTAL provides access to health information and secure messaging tools and does not replace the advice and care of your doctor. The information and means of communication provided through this service are for non-emergent and non-urgent purposes only. For medical emergencies, please call 911 or go to the nearest hospital emergency room.

MYCHART PATIENT PORTAL provides you with access to portions of your electronically stored health information. This health information is not a copy of your complete medical record, nor does it necessarily provide a complete representation of your health status. It does, however, give you a secure method to request a non-emergency appointment or to request prescriptions or refills of existing medications.

Secure messaging should not be used for medical advice. Your secure messages may be viewed and read by your doctor, other health care providers, administrators and/or others who work with your doctor or in your doctor’s practice. Therefore, you may not want to use MYCHART PATIENT PORTAL for sensitive information you wish to discuss directly with your doctor as these secure messages may become part of your permanent medical record.

To provide you with resources, MYCHART PATIENT PORTAL may contain links to CAPE COD HEALTHCARE and non-CAPE COD HEALTHCARE Internet websites or services. Unless we state otherwise, providing a non-CAPE COD HEALTHCARE link does not mean the link may be of benefit to you or that it is approved or endorsed by CAPE COD HEALTHCARE in any way.

Not all of your doctors and health care providers use MYCHART PATIENT PORTAL, so they may not be aware of information that is included in your profile. It is your responsibility to advise your doctors and health care providers about other physicians, testing and treatments related to your health of which they might be unaware.

You must be at least 13 years of age to use MYCHART PATIENT PORTAL.

Security

If you are a patient of a CAPE COD HEALTHCARE affiliate, you have certain rights associated with your health information. These rights are established under state and federal law. For a more complete description of a patient’s rights under state and federal law, please refer to the CAPE COD HEALTHCARE Notice of Privacy Practices, which can be found by clicking here.  We may disclose personal information in accordance with the Health Insurance Portability and Accountability Act (HIPAA), applicable state law and CAPE COD HEALTHCARE’s Notice of Privacy Practices

In using this service, you agree that all communication through MYCHART PATIENT PORTAL will be about you and your own health.  Once you’ve created a unique username and password, please DO NOT SHARE them with anyone. Also, you must immediately report:

  • Any inappropriate access to your account
  • Any loss or disclosure of your username or password
  • Any suspected or actual inappropriate access to or disclosure of treatment-related information or patient-related files

 The security of any Wi-Fi used to access your record through MYCHART PATIENT PORTAL or otherwise is your responsibility.

CAPE COD HEALTHCARE monitors MYCHART PATIENT PORTAL including, from time-to-time, frequency of usage, issues with usage and other specific usage, and reserves the right to revoke your access to MYCHART PATIENT PORTAL at any time in its sole discretion. You will be notified if we discontinue your access to MYCHART PATIENT PORTAL.

MYCHART PATIENT PORTAL is a private internet application provided and maintained by and proprietary to CAPE COD HEALTHCARE, on behalf of itself and its subsidiaries and their affiliated providers. Unless we have specifically authorized you to access MYCHART PATIENT PORTAL, it is a violation of state and federal law for you to do so.
